What to do before you are due

Pre-Registration should take place well before you baby's due date so you can take advantage of the services available at Your New Life Center.

You can access our on-line non-secure page, since this is a non-secure page, you could still fill it on line, print it and either fax it to us, or bring it with you to be pre-registered.

Insurance plans Palms West Hospital accepts a number of insurance plans.  Please refer to your insurance manual or click here to see a sample of carriers we do accept.

Get your overnight bag ready.  Babies may be born prior to the scheduled delivery date.  Therefore, we recommend that you have an overnight bag packed and ready to go well in advance.

Listed below are some of the items you should bring:

  • Nightgowns, underwear and nursing bras if you plan to nurse
  • A comfortable robe and slippers
  • Toiletries and cosmetics
  • A comfortable going home outfit for you and your baby, including a receiving blanket for your baby
  • A list of important phone numbers and information such as your social security number
  • Your baby book

Select your baby's name

It is hard to choose, but it is the law.  According to Florida Law, your baby's name must be registered within five days of birth.  As the date draws close, it can grow more and more difficult to decide.  Begin thinking now about names for boys and girls.

Once your baby is born, a representative from our Medical Records Department will visit you to register your baby's name.  Birth certificate and social security number worksheets will be given to you the day after your baby is born.  You should complete the forms and give them to your nurse before you go home. 

If you are not married to the father of your baby but want his name on the birth certificate as the legal father, the state requires the father to sign the back of the birth certificate.  The father will be required to show photo identification.

Once we have it, we will process the official birth certificate with the State of Florida.

Choose a pediatrician. Our obstetrics staff will need the name and phone number of the pediatrician you have selected for your baby so he/she may be notified when your baby is born. If your pediatrician is not on staff at Palms West Hospital, you may use a pediatrician on call during your hospital stay.
